This Healthcare Quality Improvement Grant funding opportunity is to assist qualifying Federally Qualified Health Centers in efforts to improve clinical quality measures in chronic disease management or preventive services and screenings. Clinics can choose to focus on one of the clinical quality measures from the list below.
Grants of up to $75,000 per year for up to a two-year period are available for quality improvement activities. The Rapides Foundation will consider only one application per organization for clinics operating in the Foundation's nine-parish service area.
